U.S. Passport Services

The Patchogue-Medford Library is a Passport Acceptance Facility for U.S. Passports. 

Passport services are available on Monday & Wednesday (evenings) & Saturdays.

Call the Main Desk at 631-654-4700 x150 to make an appointment, or make an appointment online.

You do not need a library card to use passport services at the library. 

The library offers passport photo services for $10 per person. 

Please visit the U.S. Department of State website to determine which forms, photo identification, and citizenship evidence you will need to bring to your appointment.

Passport fees paid to the U.S. Department of State are by check or money order only.
