Apply for / Renew Your Library Card Online

Library cards are available for non-residents who:

• Are owners of a business located in the library district.
• Are employees of a business or school located in the library district.
• Own assessable property in the library district.

Teens in Grades 9-12 can apply for a library card themselves, using a Patchogue-Medford school ID card or other proof of residence.

OR, a parent/guardian can apply for them. The parent/guardian will need to provide proof of residency.

Teens in Grades 6-8 can apply for a library card themselves, using a Patchogue-Medford school ID card. Please note that this card is limited to Patchogue-Medford library.

To receive a full-access card, a parent/guardian will need to apply for them. The parent/guardian must provide proof of residency.

• Current NY State Driver’s License or Learner's Permit with physical address
• Current Government issued picture ID with name and address

• Automobile registration
• Automobile insurance
• Bank statement
• Cancelled check
• Lease
• Utility or credit card bill
• Voter registration postcard

Please upload an image of one of the following:

• Proof of ownership of a business in the library district.
• Proof of employment at a business or school in the library district.
• Proof of property ownership in the library district.
